  • Mohon laporan ini disampaikan setiap bulan selama pengamatan hilal (Rukyatul Hilal) dengan mengisi formulir di bawah yang telah disediakan walaupun hujan, mendung ataupun sebab lain sehingga hilal tidak bisa terlihat/dirukyat.

  • Item bertanda (*) harap diisi sesuai data pelapor menggunakan huruf balok.

  • Waktu pelaporan disesuaikan dengan Zone waktu setempat (WIB, WITA, WIT)

  • Formulir sengaja menggunakan Bhs. Inggris untuk melatih penyesuaian thd. Rukyat Global dan pengisian boleh menggunakan Bahasa Indonesia.
